Implant placement in the maxillary anterior region presents unique challenges due to the anatomical characteristics of the anterior maxilla and the buccal bone’s natural resorption pattern following tooth extraction. Partial Extraction Therapy (PET) – which includes root submergence, socket shield and pontic shield techniques – offers a biologically-driven approach to preserve buccal bone and maintain hard and soft tissue architecture. This presentation will review the principles and clinical application of PET, with a primary focus on the socket shield technique. Indications, contraindications, surgical protocols, potential complications and alternatives to PET will be discussed. A series of clinical cases, ranging from single-unit restorations to full arch applications, will illustrate the versatility and predictability of PET in enhancing esthetic and functional implant outcomes.
LEARNING OBJECTIVES:
- Recognize the Aesthetic and Functional Benefits of Partial Extraction Therapy (PET): Understand how PET techniques preserve buccal bone and soft tissue for improved esthetic outcomes.
- Master the Socket Shield Technique for Single and Full-Arch Cases: Learn step-by-step protocols for predictable PET execution.
- Identify Indications and Contraindications for PET: Evaluate when root submergence, socket shield or pontic shield techniques are appropriate.
- Manage Surgical Complications in PET: Explore common challenges and strategies to address them effectively.
- Apply PET to a Range of Clinical Scenarios: Gain confidence integrating PET modalities into single-unit and full arch implant workflows.
- Understand the Role of Implant Design in Socket Shield Success: Learn how implant macro- and micro-design considerations influence stability, integration and aesthetic outcomes in PET cases.
- Learn about alternative techniques to utilize when PET is not indicated.
